Home and Community Support offers a range of flexible services covering home nursing, home assistance and other specialised care including dementia support.

Getting started

When it comes to getting extra care of support, it can be difficult to know where to begin. We’d love to help you get started, with the services and support that’ll help you live the life you want.

If you are considering a Home Care Package, we can assist with any of the steps below. For other funding options please contact our helpful team.

Step 1 – Complete an Aged Care Assessment

When considering your aged care options, you’ll first need to arrange an assessment through My Aged Care or call 1800 200 422. They’ll chat with you about your day-to-day routine, physical and mental health, and what’s important to you.

Step 2 – Find out your assessment outcome

After your assessment, the assessor will let you know if you’re eligible for care and the level of services you are approved to receive.

Depending on your situation, this may be a Home Care Package Level 1-4 for ongoing needs, Short-Term Restorative Care or Commonwealth Home Support Programme.

When you are approved for a Home Care Package, you will be placed on the national queue for care.

Step 3 – Work out the cost

Depending on your personal circumstances, you may be asked to contribute to the cost of your care.

If you're approved for a Home Care Package, you’ll need to complete a financial assessment with Services Australia to determine if you need to pay an income tested care fee.

Step 4 – Select your aged care provider

When you are approved for home care services, you can nominate the provider to deliver your care and services in the place you call home.

Our friendly Home and Community Support team would love to talk with you about your specific needs and what matters to you most. Call 1300 22 11 22 to learn about the range of services available to you.

Step 5 – Accept your Home Care Package

When your Home Care Package is assigned, you will receive a letter from My Aged Care. You will then have 56 days to choose a home care provider.

Bolton Clarke will work with you to select services that meet your needs, develop a care plan and confirm the costs. These details will be set out in a Home Care Agreement.

Before signing an agreement, it’s a good idea to seek independent advice from a specialised aged care advisor.

Step 6 – Start your care plan

When it’s time to begin your care plan with Bolton Clarke, our team will support you to age positively and live independently at home for longer.

As your needs change, we’ll work with you to provide holistic, best-practice care.
